Enfermedad Celiaca: generalidades sobre presentación, diagnóstico y tratamiento

Resumen

La celiaquía es una enfermedad con una presentación clínica amplia originada por la “malabsorción” del gluten a nivel intestinal. Su método de diagnóstico es endoscópico, tomando una biopsia, sin embargo, con el pasar de los años se han identificado exámenes de sangre llamados serologías y genéticos para evitar el procedimiento endoscópico invasivo. Estos métodos de diagnóstico nuevos se utilizan actualmente con mayor frecuencia. La asociación Americana de Gastroenterólogos los incluye en su guía de diagnóstico para la enfermedad celiaca. La biopsia dará el diagnóstico e indica el grado de severidad de atrofia a nivel de vellosidades clasificándola de manera histológica. El tratamiento para esta enfermedad se centra en la dieta libre de gluten, lo cual requiere de educación en términos de nutrición y en suplementación nutricional, en casos de haber evidenciado deficiencias por dicha malabsorción. No existen al momento tratamiento farmacológico, solo en casos graves se ha requerido de utilizar esteroides o inmunomoduladores.

Abstract

The celiac disease is a sickness with a wide variety of symptoms that is produced because of lack of absorption of gluten in the intestines. It is diagnosed threw endoscopy, taking a biopsy, nevertheless, with the time that has passed there has been blood tests called serological testing and genetic testing that can help avoid the invasive endoscopy. These new methods of diagnosis are being used with more frequency, the American association of gastroenterologists are including them in their guide for diagnosing celiac disease. The biopsy will make the diagnosis and will indicate the severity of the atrophy of intestinal villi giving the diagnosis with a histological classification. The treatment for this disease is centered in a gluten free diet, which requires education in terms of nutrition and nutritional supplements in case of having notice nutritional deficiencies because of the lack of absorption. It does not exist to this moment pharmacological treatment, only in severe cases it is required the use of steroids and inmunomodulators.
